If you would like to be sub-contracted by HSW, please download and fill out the prequalification form to the right. Print and sign the completed form and send it back per the submission instructions on the last page For electronic submissions, please scan a signed copy of the completed form and send us an email attachment to the designated recipient listed in the form itself.
